The Foreign Medical Graduate Licentiate Regulations (FMGL-2021) with its further advisories and amendments are the standard rules governing the mandatory requirements for registrations and independent medical practice in India after completion of medical studies.
The 12 thumbrules of FMGL for Foreign Medical Graduates.
When you select a country or an MBBS program or its equivalent outside India, the following critical points to be kept in mind while selecting a country.
You must have a valid NEET score while taking admission to the first year of your medical program for students, aspiring to study medicine outside India, the NEET validity is 3 years from the time they cleared the NEET Exam. Many institutions outside India(Especially in European countries) can offer you admission to a 6year bachelor program even without a valid NEET score.
But if you want to take a registration under any medical council in India after your MBBS studies, it is imperative to have a valid NEET score when you take admission to the first year of your MBBS or its equivalent program. Otherwise, you will not be permitted to appear for the NEXT exam or the registration in Idia for independent medical practice after your studies.
As per FMGL Clause 4a(i), course duration of the MBBS program(or its equivalent bachelor program in medicine) must be of 54 months or more. That means your course duration should be same as the course duration of the MBBS program in India.
Countries which are following the US Pattern of medical education such as the USA,Canada,Australia,New Zealand,Philippines, Caribbean Islands,etc. are following a 2+4 pattern of medical education wherein 02 years of pre-medical course to be done inorder to qualify for the 04 year undergraduate medical program.
Although it is written as MD (Doctor of Medicine) it is equivalent to a basic medical degree like MBBS.
As per 4 a (ii) of FMGL regulations 2021, a foreign medical graduate must complete a 12 months internship from the same institution or from the medical facility attached to the same university.
That means the medical institution where you are studying medicine does not have sufficient or own facility to do clinical practice, your degree will be considered invalid.
There are several countries do not allow students to do internship in their country although they offer MBBS or its equivalent program to international students. Such countries give open permission to do internship in their home country. But as per the regulations of NMC, if the internship is not done at the same medical institutions, you may face conformity issues while you attempt to do registration under the medical councils in India.
Many small countries offering MBBS or its equivalent 6 year medicine undergraduate programs do not have sufficient infrastructure or hospital facilities to arrange internship to all the students. Normally they do some tie-up with some hospitals and do clinical trials.
But such makeshift arrangements could affect your opportunities for proper clinical exposure which is required for passing both phases of NEXT exam in India or may hamper the registration requirements put forth by the National Medical Commission.
The internship, a foreign medical graduate undergo must be in the same pattern as the MBBS program in India.
That means the internship in your chosen country must comprise 54 months’ theoretical studies and 12 months Compulsory Rotational Medical Internship-CRMI.
Many European Countries and CIS countries have a merged curriculum of 5th and 6th year of their studies. Under this pattern, the above criteria by NMC are not met with and you may require an additional one year of internship after studies in such countries.So must check with the NMC guidelines before you take admission in those countries.
The FMGL guidelines further states that the 12 months internship must be in the same pattern of MBBS program in India with ample practical exposure to the specific subjects including Community Medicine, General Medicine, Paediatrics, General Surgery, Anaesthesia, Obstetrics and Gynaecology, Orthopaedics, Otolaryngology, Ophthalmology, Dermatology, Emergency or Casuality Services, Lab Services and their sub-specialities.
This clause tells us the importance the NMC is giving to the clinical exposure the students have to acquire during the 12 months internship when the students study for a medical degree program in a foreign country.
So, you must choose a study destination which is a developed country having all the amenities to attain the desired clinical exposure.
As per the clause 4a(iii) of FMGL 2021, NMC will accept a medical degree which is taught fully in English medium. If the program is conducted in bilingual mode, then your qualification will be considered invalid.
As per 4a(iv) of FMGL regulation, the MBBS program in your chosen destination/country must have all the compulsory subjects mentioned under Schedule-I of FMGL regulations.
That means the curriculum they follow must assimilate all the 19 subjects in the pre-clinical, para-clinical and clinical phase of MBBS program in India.
The FMGL regulation by the NMC insists that the student doing their MBBS or its equivalent program outside India must secure a medical license for independent medical practice issued by the competent authority (having the same status as NMC in that country) from the studying country.
There are only few countries in world who grant license to foreigners or international students and allow them to do independent practice in their country.
Without a medical license from the studying destination, a foreign medical graduate will not be permitted to appear for NEXT exam or registration for independent medical practice in India.
Countries such as China, Russia, Kazakhstan, Nepal, Bangladesh, etc. are giving medical practice license and registration for Indian students. Few countries such as Georgia, Belarus,Egypt etc. also give license to international medical students after completing post-graduate medical residency/compulsory medical internship of two years after completing Medical studies.
Many European countries (which are part of EU)are granting license to international students upon completion of Medical studies or as such there is no specific requirements for qualifying exam to obtain a Medical license in those countries who have done there Medical studies from those countries.
A few countries in the world are following reciprocity in licensing matters. These countries give license for medical practicingto foreigners only if their nationalities are granted license in India. But in our country Medical Practicing license is not given to foreign citizens. So eventually the chances of getting Medical Practicing license after your studies may not be possible. So, you must be careful while choosing such countries.
Schedule– I/2(ii)b of the FMGL Regulation insist that your curriculum of the medical program should be displayed on the website of the Medical institutions or college for verification /review by the appropriate authorities when the students join for the Medical Program in the case of Universities and Medical colleges outside India.
Clause 4d of the FMGL Regulation mention the mandatory requirements of clearing the NEXT Exam which expected to be introduced this year in place of foreign Medical Graduate Exam(FMGE).
So when a student selects a college for Medical studies outside India, it is his duty to make sure that the chosen institution /Country complies all the conditions laid down by the NMC by means of Foreign Medical Graduate Licentiate regulations in order to successfully register in India after completion of their MBBS Course.
As per Schedule I/2 (iii) of the FMGL Regulations, a student must complete the entire duration of the MBBS (or its equivalent) programfrom the same institution or university. Transfer to any other institution or university is not permitted.
The entire tenure of the MBBS program as well as the internship has to be completed from the same institution in which the student is doing his medical education outside India.
That means transfer from one institution to another for any reason is not allowed at any point of studies including the internship.
As per Schedule I/2 (iv), the total duration of the foreign medical course must be completed within a period of 10 years from the date of joining. As per this clause, a student pursuing MBBS (or its equivalent) program outside India must complete his registration within 10 years of time. This includes the time for both stages of the NEXT exam and mandatory 12 months internship in India.
So, if you are looking forward to admission outside India for MBBS, you must ideally select countries with the shortest course duration (but it must be well above 54 months), having separate internship of 12 months after his studies and the countries where you can obtain a practice license in order to conform to the conditions of FMGL 2021.
